Informed Consent Release Form

Nature of Treatment

  • Purpose: Tattooing is a cosmetic procedure that involves injecting pigment into the dermal layer of the skin using needles to create permanent designs or markings.

  • Procedure: A sterile needle is used to apply ink beneath the surface of the skin. The tattoo artist will follow design specifications as agreed upon during the consultation.

  • Expectations: Tattoos are intended to be permanent, but some fading may occur over time due to factors such as sun exposure, skin type, ink color, and location on the body.

  • Touch-Ups: Depending on healing and individual skin response, touch-up sessions may be required to maintain the appearance of the tattoo.

Risks

  • Possible Side Effects: As with any procedure that involves breaking the skin, there are inherent risks, including but not limited to:

    • Pain, swelling, redness, bleeding, and tenderness

    • Infection if aftercare instructions are not followed properly

    • Allergic reactions to tattoo ink or other materials

    • Scarring or keloid formation, particularly in individuals prone to such skin reactions

    • Unintended changes in tattoo appearance during healing or due to skin conditions

  • Medical Conditions: Certain medical conditions or medications (such as blood thinners or immune disorders) may increase the risk of complications. It is important to disclose all relevant health history to the tattoo artist before the procedure.

Medical Management

  • Practitioner Qualifications: Tattoos will be applied by a licensed tattoo artist trained in proper hygiene, sterilization, and application techniques, in compliance with local health department regulations.

  • Consultation: A consultation may be conducted to discuss the design, placement, and potential risks. Clients are encouraged to ask questions and express any concerns before the procedure begins.

  • Medical Disclosure: Clients must inform their artist of any medical conditions, allergies, or skin sensitivities prior to each session. Failure to do so may increase the risk of complications.

  • Records: The studio may maintain documentation of the procedure and any disclosures made by the client. These records will be stored confidentially in accordance with applicable privacy laws.

Pregnancy and Health Conditions

  • Advisory: Tattooing is not recommended during pregnancy or breastfeeding due to potential unknown risks. Clients must notify the artist if they are pregnant, may be pregnant, or are nursing.

  • Medical Clearance: In some cases, clients may be asked to obtain medical clearance from a physician prior to the procedure.

Aftercare Instructions

Depending on your artist and skin sensitivities, you will be provided with one of the following aftercare methods:

DERM SHIELD AFTERCARE:

  • Leave the Recovery Derm Shield on for 4-5 days. It will protect your tattoo during it's healing process. After the 4-5 days, gently peel away the Derm Shield by pulling down from the corners. This will work best while under warm, running water. Once it has all been removed, wash your tattoo with Dial antibacterial soap and warm water. Use unscented lotion after washing for the remainder of the healing process (Day 5-14).

  • If at any point in the first 5 days the Derm Shield begins to peel or rip, allowing liquid or air into the tattoo, remove the Derm Shield and continue until day 5 washing your tattoo with Dial antibacterial soap 2-3x/day and applying a VERY THIN layer of Aquaphor after patting dry with a paper towel.

SARAN WRAP/BANDAGE AFTERCARE:

Day 1:

  • Leave wrap on for 3-4 hours

  • Remove & wash with antibacterial soap

  • Pat dry with paper towel

Day 2-5: Morning/Noon/Night

  • Wash with antibacterial soap

  • Pat dry with paper towel

  • Apply a VERY THIN amount of ointment

Day 6-14: Morning/Noon/Night

  • Wash with antibacterial soap

  • Pat dry with paper towel

  • Apply a VERY THIN amount of unscented lotion (Lubriderm, Eucerin, Nivea, etc.)
